If you created an account on the Call-On-Doc, Inc. website while in California commencing on November 7, 2023, to the date of Final Judgment; or were registered user with Call-On-Doc and started to book appointments for medical care in California (regardless of completion of the medical appointment) commencing on November 7, 2021, to the date of Final Judgment, then you may be a Settlement Class Member. Excluded from the Settlement Class are: (1) the Judge in this case, and the Judge’s family and staff; (2) Call-On-Doc and its officers, directors, and related companies; and (3) anyone who validly excludes themselves from the Settlement.

If you are a Settlement Class Member and wish to receive a cash payment then you must submit a Claim Form by August 29, 2026.

All Class Members are eligible to submit a claim for a cash payment. Class Members will receive up to $20.00.

The deadline to submit a Claim Form online is August 29, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than August 29, 2026.
